Coaches Code of Conduct currently under revision Players Code of Conduct All players within our club are expected to abide by our code of conduct. Coaches will be distributing these at the beginning of the season to all travelling teams. Medical Forms Need a blank medical form for one of your players? Download it here. Police Checks All adults that have any contact with the youth in our club, either coaching or at practices, require a police check every two years. Forms can be downloaded here and submitted to our Volunteer Screening Coordinator. Note that form must be printed double-sided (ie both sides onto a single piece of paper). The OPP require it that way.
